ATTENTION

Effective July 1, 2018, New York State Education Law requires:
 

1. NEW ENTRANTS of the Wappingers Central School District must have an updated physical exam in addition to all vaccinations on file.
2. ***9th and 11th grade students *** Each student in grades Kindergarten, 1, 3, 5, 7, 9 and 11 must have a physical examination.

Please send a copy of your child’s physical examination, performed by your private physician on the attached MANDATED NYS School Health Examination Form, to the Health Office by October 1.

Parents are encouraged to have their child examined by their family physicians.  Results of the examinations must be noted on health forms furnished by the District, signed by a doctor and returned to the Health Office by October 1st.
