When changing an address, the following is required before the change will be put into the system.

TWO PROOFS OF RESIDENCY: (The following are acceptable proofs of residency)

1. A lease or mortgage statement
2. An electric/telephone/utility bill
3. An IRS statement
4. Insurance bill
5. Garbage bill
6. Fuel delivery
7. Water/Sewer bill

Upon receipt of the proofs, the system will be updated for transportation and contact purposes.
